BEFORE YOUR APPOINTMENT
 

In general, appointments are scheduled on a weekly or bi-weekly basis, but are also offered on an “as-needed” basis.  Call to schedule your appointment directly with me. 

If you must cancel an appointment, please do so a full 24-hours in advance of the session.  Missed or cancelled appointments with less than 24 hours notice will be charged at your usual fee. 

Please download all documents (using links below) and fill out completely prior to your session. If you can't access these documents through the website, please call and they can be faxed or sent by mail. Bring these documents with you. Otherwise, a portion of your first session will be spent completing these documents.

FEE and PAYMENT INFORMATION
 
The fee for family, couples, children and individual therapy is $85 per fifty minute session. A discounted rate may be available for those who can demonstrate genuine financial need. Please discuss this with me directly before our first session.
You will be asked to make the payment at the beginning of each session.  I accept cash, check, debit, credit cards, and PayPal.  Make your check payable to “Amy Sutton, MA”.   There is a standard $25 fee for all checks returned due to insufficient funds.  PayPal payments can be made prior to the session through this website.  PayPal payments should be sent to counseling@milestonespsychotherapy.com. Credit card payments can be made prior to the session at the website or in the office at the time the service is rendered.  I do not accept insurance.
